ABOUT THE CENTER OF HEARING AND SPEECH MEDINCUS
The Center of Hearing and Speech MEDINCUS is an otorhinologic network of 18 medical centers based in Poland and 7 facilities abroad (Senegal, Kyrgyzstan, Kazakhstan, and Ukraine). The MEDINCUS Hospital is located in Kajetany, heart of the Polish school of otolaryngology.
Our team includes doctors and surgeons who specialize in the diagnostics and treatment of hearing loss and deafness in children and adults, as well as diseases of the throat, larynx, trachea, sinuses, and nose.
We perform over 130 different types of surgeries in the field of otolaryngology.
MEDINCUS is a modernly equipped department with an operating suite, which includes two operating rooms.
Our patients have the option to stay at the on-site Hotel Park Kajetany, which provides full-board dining

The MEDINCUS Center in Kajetany specializes in surgical treatments of the throat, ear, nose, sinuses, and larynx. We also provide advanced cochlear implants, middle ear implants, and bone conduction implants. One parent can stay with the child after the operation.

HOTEL FOR OUR PATIENTS
All our patients and their accompanying persons have the possibility to stay in our hotel during the diagnosis, treatment and rehabilitation. There are three types of rooms: standard, superior and apartment. We suggest single and double rooms with the option to add one more bed. Appartment room is an ideal proposition for big family long-stay. All hotel costs are included in the price for our CI patients.

Kajetany is located in 20 km from Warsaw. There are several options to get to Kajetany*:

- - by Warsaw city bus of the zone 2 from the bus station on Al. Krakowska to town Nadarzyn, from bus station in Nadarzyn there is only 1,5 km to Kajetany and the MEDINCUS facilities

We take care about the insurance documents for our patients. We check the possibility for you to use your national insuarance coverage for some operations (like CI for children)
We provide language support to all our patients in English, Russian, French, Spanish, Italian languages. We do plan all patient's visit with full understanding that not all our foreing patients speak Polish.

Pani/Pana dane osobowe przetwarzane będą przez Podmiot Leczniczy na podstawie art. 9 ust. 2 lit h) RODO, w celu diagnozy medycznej, zapewnienia opieki zdrowotnej, diagnozy medycznej i leczenia, zarządzania systemami i usługami opieki zdrowotnej na podstawie właściwych przepisów prawa: w powiązaniu z art. 3 ust. 1 ustawy z dnia 15 kwietnia 2011 r. o działalności leczniczej oraz art. 24 i następne ustawy z dnia 6 listopada 2008 r. o prawach pacjenta i Rzeczniku Praw Pacjenta, które to cele obejmują w szczególności przetwarzanie danych związane z: rejestracją Pani/Pana w placówce Podmiotu Leczniczego, świadczenia Pani/Panu świadczeń zdrowotnych, zapewnienia jakości udzielania świadczeń, zapewnienie ciągłości opieki zdrowotnej, w tym w procesie koordynacji udzielania świadczeń co obejmuje m.in. przypomnienie o terminie realizacji świadczenia zdrowotnego, potwierdzenie wizyty lub odwołanie wizyty, zapewnienia udokumentowania Pani/Pana stanu zdrowia oraz udzielonych Pani/Panu świadczeń zdrowotnych, poinformowanie o możliwości wzięcia udziału w ankiecie badania jakości udzielonego świadczenia, a w przypadku udzielania świadczeń opieki zdrowotnej finansowanych ze środków publicznych - także na podstawie art. 2 oraz art. 49 i nast. ustawy z dnia 27 sierpnia 2004 r. o świadczeniach opieki zdrowotnej finansowanych ze środków publicznych, a w przypadkach, o których mowa w art. 12 i nast. ww. ustawy i na podstawie wskazanych przepisów szczególnych, w celach weryfikacji Pani/Pana uprawnień do uzyskania świadczeń opieki zdrowotnej finansowanych ze środków publicznych, a także w celach rozliczenia tych świadczeń z Narodowym Funduszem Zdrowia lub innym podmiotem finansującym te świadczenia.
- 5. Ponadto, w koniecznym zakresie, Pani/Pana dane osobowe mogą być przetwarzane przez Podmiot Leczniczy na podstawie prawnie uzasadnionego interesu Podmiotu Leczniczego w celu dochodzenia ewentualnych roszczeń związanych z udzielonym Pani/Panu świadczeniem zdrowotnym.
- 6. Odbiorcą Pani/Pana danych osobowych mogą być osoby i podmioty wskazane w art. 26 ustawy z dnia 6 listopada 2008 r. o prawach pacjenta i Rzeczniku Praw Pacjenta oraz w przepisach ustaw szczególnych, a także dostawcy usług i rozwiązań technicznych/organizacyjnych (dostawcy usług IT, firmy kurierskie, pocztowe, dostawcy sprzętu diagnostycznego itp.), dostawcy usług prawnych i doradczych (kancelariom www.medincus.pl v. 03/20 2 www.medincus.pl v. 03/20 prawnym, firmom windykacyjnym itp.) oraz osoby upoważnione przez Panią/Pana. W przypadku udzielania świadczeń opieki zdrowotnej finansowanych ze środków publicznych odbiorcą Pani/Pana danych osobowych będzie Narodowy Fundusz Zdrowia lub inny podmiot finansujący świadczenia opieki zdrowotnej.
- 7. Pani/Pana dane osobowe nie będą przekazywane do państwa trzeciego (tj. państwa nienależącego do Europejskiego Obszaru Gospodarczego).
- 8. Pani/Pana dane osobowe zawarte w dokumentacji medycznej będą przetwarzane zgodnie z wymogami art. 29 ust. 1 ustawy z 6 listopada 2008 r. o prawach pacjenta i Rzeczniku Praw Pacjenta (Dz.U. z 2017 r. poz. 1318), tj. przez okres 20 lat, licząc od końca roku kalendarzowego, w którym dokonano ostatniego wpisu, z wyjątkiem: • dokumentacji medycznej w przypadku zgonu pacjenta na skutek uszkodzenia ciała lub zatrucia, która będzie przechowywana przez okres 30 lat, licząc od końca roku kalendarzowego, w którym nastąpił zgon, • dokumentacji medycznej zawierającej dane niezbędne do monitorowania losów krwi i jej składników, która będzie przechowywana przez okres 30 lat, licząc od końca roku kalendarzowego, w którym dokonano ostatniego wpisu, • zdjęć rentgenowskich przechowywanych poza dokumentacją medyczną pacjenta, które będą przechowywane przez okres 10 lat, licząc od końca roku kalendarzowego, w którym wykonano zdjęcie, • skierowań na badania lub zleceń lekarza, które będą przechowywane przez okres 5 lat, licząc od końca roku kalendarzowego, w którym udzielono świadczenia zdrowotnego będącego przedmiotem skierowania lub zlecenia lekarza, a przypadkach, gdy świadczenie nie zostało udzielone z powodu niezgłoszenia się pacjenta w ustalonym terminie – przez okres 2 lat, chyba że pacjent odebrał skierowanie, • dokumentacji medycznej dotyczącej dzieci do ukończenia 2 roku życia, która będzie przechowywana przez okres 22 lat. Pani/Pana dane osobowe przetwarzane w oparciu o cel wskazany w pkt 5 powyżej będą przetwarzane przez okres konieczny z uwagi na cel przetwarzania, nie dłużej niż przez do czasu przedawnienia roszczeń.
- 9. Posiada Pani/Pan prawo dostępu do treści swoich danych oraz prawo ich sprostowania, usunięcia, ograniczenia przetwarzania, prawo do przenoszenia danych, prawo wniesienia sprzeciwu, prawo do cofnięcia zgody w dowolnym momencie bez wpływu na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej cofnięciem, z zastrzeżeniem jednak zasad gromadzenia i przetwarzania danych w dokumentacji medycznej, o których mowa w rozdziale 7 ustawy z dnia 6 listopada 2008 r. o prawach pacjenta i Rzeczniku Praw Pacjenta oraz w przepisach ustaw szczególnych.
- 10. Posiada Pan/Pani prawo wniesienia skargi do Prezesa Urzędu Ochrony Danych Osobowych (PUODO), gdy uzna Pani/Pan, iż przetwarzanie dotyczących Pani/Pana danych osobowych narusza przepisy obowiązującego prawa.
- 11. Podanie przez Pana/Panią danych osobowych w części w jakiej wynika z mocy przepisów prawa tj. ustawy o działalności leczniczej i ustawy o prawach pacjenta i Rzeczniku Praw Pacjenta jest obligatoryjne. Brak podania danych osobowych będzie skutkował możliwością odstąpienia przez Podmiot Leczniczy od udzielenia świadczeń zdrowotnych.
- 12. Pani/Pana dane nie będą przetwarzane w sposób zautomatyzowany, w tym nie będą podlegały profilowaniu.
